Course Outline:

Restorative Dentists are finding themselves at the crossroads of Esthetic and Implant Dentistry.   There is an explosion of concepts with very little scientific evidence.  The Dilemma of whether to save teeth or to replace them with implants becomes a challenge due to predictability, reproducibility, and often to financial considerations.  Rather than custom create treatment plans, we are forced into treatment driven options – “we have a solution, now let's fit all problems into this” has led to radically adopting concepts such as “ All-on-four”, “Immediate Smile” “Teeth in a Day”.  

 

Objectives:

This course is designed to help the practitioner with evidence-based concepts of treatment plans that rely heavily on comprehensive diagnosis.  A decision tree will be illustrated with objective analysis of esthetic and functional demands of the patient and the formulation of a rational plan would be facilitated.  Fundamentals of restorative diagnosis with contemporary occlusal concepts will be reviewed.  Case examples will elaborate the predictable nature of traditional crown and bridge restorations and when they perform better than implants.  A logical sequence of utilizing natural teeth and implants to get ideal esthetic and functional outcomes will be taught.

1. Diagnosis and Treatment planning of the Missing anterior tooth
      a. When to save and when to replace
      b. Crown and Bridges vs. Implants vs. removable partial dentures
            i. Is there a therapeutic hierarchy?
            ii. Factors to consider when replacing a tooth in the Esthetic Zone
      c. Long term outcomes of fixed restorations
      d. Material considerations in the Esthetic Zone – PFMs vs All ceramic restorations
            i. Do all ceramic restorations outperform PFM crowns?
            ii. Zirconia as a restorative material –
            iii. Cements and its influence on shade
            iv. Choice of luting agents

2. Restoration of the Worn Dentition
      a. Etiology of wear
      b. Fundamentals of Occlusal concepts
      c. Vertical dimension of Occlusion
      d. Treatment strategies when dealing with occlusal disease
            i. Conformative treatment
            ii. Reconstructive Treatment
            iii. Staged treatment
      e. Should one increase the vertical dimension or perform crown lengthening?

3. Treatment of the terminal dentition
      a. Concepts of Staged treatment plans vs immediate loading
      b. Fixed vs Overdentures – what choices do we make and how do we sequence it.
      c. The immediate load options with dental implants – immediate placements and immediate function.
      d. When to choose a Fixed Ceramic FPD over All on Four hybrids.
      e. Protocols for Full arch implant supported restorations
      f. Prosthetic options in complete arch implant restorations.
            i. How many implants does a full arch require?
            ii. Are screw retained restorations better than cement retained?
            iii. What are the best cements for implants

4. Failures in Rehabilitation
      a. Avoiding complications with full mouth rehabilitation
      b. Management of failures in reconstruction
